Information about our partners

All Together Better Sunderland’s main aim is to help improve and join up health and social care services to keep the people who need the most support, independent and as well as possible.

We have a range of organisations making up the All Together Better partnership. Some oversee delivery of the project; making sure every we plan is done properly and that the budget is spent wisely. Others provide important services directly – like nurses; social care teams and community and voluntary organisations. Other organisation we work with guide and advise us.

All of these organisations work in partnership to make sure All Together Better is the best service it can be for local people. Below is a list of some of the organisations and links to important documents related our aim:

Provides a vast range of public services and manager social care in the city.

Provides direct support to people who look after family members or others who can’t look after themselves (carers).

Sunderland Care and Support

Provides a range of important social care service such as reablement and specialist equipment supply but also importantly is the home of the Recovery at Home team. Lots of information is available about the service is on their website.

Northumberland, Tyne and Wear NHS Foundation Trust

Northumberland, Tyne and Wear NHS Foundation Trust is one of the largest mental health and disability Trusts in England, serving a population of approximately 1.4 million, over 60 sites across Northumberland, Newcastle, North Tyneside, Gateshead, South Tyneside and Sunderland. The main charity looking after older people and responsible for patient and public engagement on the project.

South Tyneside Foundation Trust

Provides and manages community health services for Sunderland. For All Together Better the trust provides frontline, specialist staff like district nurses and therapists Useful links relating to All Together Sunderland.

City Hospitals Sunderland

Provides and manages a vast range of acute (hospital) services in the city. For All Together Better the trust provides specialist staff like pharmacists and geriatricians (specialist in care for older people).

Sunderland GP Alliance

Sunderland GP Alliance exists to help meet the challenges of today’s healthcare. We live longer, with complex health issues and, by working collaboratively with a range of partners, the Alliance provides proactive and sustainable primary care for local people in the city. They lead on some specific pieces of work for All Together Sunderland including being the primary project lead on Enhanced Primary Care.
